GP referrals

Some of our health professionals require a referral from your GP to ensure you receive advice and care appropriate to your current health needs.

To help speed up the process, you can request a referral directly from your GP. Simply download and print our referral form, ask your GP to complete the relevant sections, and have it sent to us by fax, email or post. Once we have received your referral, we will arrange a suitable time and location for your consultation.
